Porcelain vases with ED street names - who makes them?

At the Artists' Open House I saw some gorgeous vases made from strips of different street signs in ED. I've scoured the programme from Dulwich Festival 2013 and can't find them, also can't remember which house they were in. I would really like to buy one now as a present. Does anyone happen to know who the artist is and how I can get in touch?
